Applying for a Urology Grant
The Oregon Urology Foundation accepts urology grant requests from organizations in the range of 10k – 25k preferably for the use of providing urologic care to patients.
Please fill out our Grant Application Form. The OUF Board of Directors will review applications at their next board meeting. Your agency will be notified via mail &/or e-mail once a decision has been made.
Submit Grant Applications to:
Oregon Urology Foundation, 2400 Hartman Lane, Suite 300, Springfield, OR 97477. Please include to whose attention the urology grant check should be written as well as the appropriate address if awarded.
